Contra Costa Health Services recognizes that some people are unable to leave their homes to get vaccinated. To help meet this need, the Health Services department is offering COVID-19 vaccines for people who are homebound. Health Services is collecting names of people wanting a home vaccine and will start to schedule appointments on May 17, 2021. Request an appointment by completing this simple on-line form, which can be submitted by an individual or his or her caregiver, or call to make an appointment at 1-833-829-2626.
